Comprehensive New Patient Visit $225

45-minute visit to establish care with Optimized Health & Wellness. Includes full review of medical, family, and social history, medication review and refills (if appropriate), focused physical exam, vitals, and InBody body composition scan. Lab orders may be placed if needed (labs not included). Ideal for those looking to transition their care or get started with a personalized wellness plan.

Follow Up Visit $75- $125

30–60 minute appointment based on complexity. Designed to review lab results, adjust treatment plans, monitor progress, refill medications, and address ongoing health concerns or goals. This visit ensures continuity of personalized care, whether for hormone optimization, weight management, or general wellness support.

Sick Visit starting at $100

For acute issues such as sinus pain, sore throat, ear pain, flu symptoms, UTI, yeast infections, mild rashes, STD testing, or medication refills. These visits are typically 20–30 minutes and focused on quick relief and appropriate treatment.

Televisit $75 (established patients only)

We offer comprehensive wellness packages for hormone optimization (for both men and women) and weight loss/metabolic reset, designed to support your health journey whether 4 months or 12 months. Each package includes lab work, detailed lab reviews, follow-up visits, prescription management, ongoing education, and direct messaging access to your provider. We have three tiers—Basic, Comprehensive, and Elite—so you can choose the level of support that best fits your needs and goals. Mobile Services

Would you prefer care without leaving the comfort of your home? Whether you're not feeling well, short on time, or simply value the convenience, we offer mobile visits for sick visits, wellness checkups, and follow-up care in select East Valley areas. This service is available for an additional $75 travel fee, with flexible scheduling to accommodate your needs. Availability is limited, so we recommend booking in advance to secure a convenient time.

In-House Labs & Diagnostic Imaging

We offer the convenience of in-house blood draws and affordable lab orders—no need to visit an outside facility. Our self-pay lab pricing is often significantly lower than traditional insurance-based costs or co-pays, making comprehensive testing more accessible. When imaging is needed, we can provide radiology orders with self-pay options that are also budget-friendly.
